We’re excited to welcome you to the Music Discovery family! Please fill out this form to register for a free trial lesson for each student interested in joining our musical journey.
Instructions:
- Fill out this form once for each student.
- If you’re requesting trial lessons for multiple family members (siblings, dad/daughter, cousins, etc.), please submit a separate form for each person.
- Free trial lessons are not free to us — your teacher is paid, whether you attend your free trial lesson or not. Please be certain to attend your free trial lesson or reschedule at least 48 hours in advance.
- We are only able to offer one 30-minute time slot per family. If you have more than one family member who wants a free trial lesson, they will share a 30-minute slot. Thank you for your understanding.
Notes:
- The “Your name” field should be filled with the name of the adult completing this form, who will be designated as the billing contact. If you prefer someone else to be the billing contact, we can make that adjustment at a later time.
- “DofB” refers to the student’s date of birth. In addition to the day and month, please include the birth year. This information helps us tailor our programs to the appropriate age group.
Important Information:
Music Discovery offers one free trial lesson per student. If you’re interested in a second trial lesson, there is a reduced cost tuition of $20, payable 48 hours in advance of your free trial lesson. This option is generally intended for students deciding between two different instruments or for those who, for any reason, missed their initial trial lesson. Thank you for your understanding.
